Internship Forms

Evans School internship forms, except for the internship waiver, are online forms. For the internship waiver you will need Adobe Acrobat Reader to view it.

• Agency/Student Contract Form
  • Describes the scope of work, your professional objectives, and the agency’s expectations
  • Must be submitted within two weeks of starting your internship
• Internship Evaluation Form
  • Describes the quality of the internship and what you got out of it
  • Must be submitted within two weeks of completing the internship
• Intern Supervisor Evaluation Form
  • Remind your supervisor to complete and submit the form
  • Must be submitted to Career Services during your last week of work
