Evans School degree project forms are available in PDF format and require Adobe Acrobat Reader to view.

• Degree Project Proposal Form (34KB PDF): Must be submitted to the Student Services office before getting a registration code
• Degree Project Sample Title Page (5KB PDF): Sample format for the title page of your degree project
• Degree Project Summary (7KB PDF): Must be submitted with your degree project
