Degree Project Forms
Evans School degree project (DP) forms are available in PDF format and require Adobe Acrobat Reader to view.
- Independent Degree Project Proposal Form (40 KB PDF): Must be submitted electronically to evansreg@uw.edu before a registration code can be given out for independent degree project credits. (Those pursuing their degree project through a degree project seminar or a public service clinic do not need to complete this form; instead, simply register for the appropriate seminar or clinic.)
- Degree Project Sample Title Page (5 KB PDF): Sample format for the title page of your degree project.
Degree Project Electronic Submission
All DPs and title pages must be submitted online via the Evans School DP archive. You will also be required to fill out a DP summary/abstract (see sample; 48 KB PDF).
Steps
- Submit final version of DP to reader
- Have reader sign a title page (see sample; 28 KB PDF) in “Approved by” section
- Create two PDFs (one of the DP and one of the signed title page)
- Submit both documents and your summary/abstract online using the Evans School DP archive.
